Parental Consent & Participation Waiver

I, hereby give my consent for my child to participate in the football coaching program offered by Johnathan Riley of LilDawg Coaching and any affiliates. I understand and agree to the terms and conditions outlined below:

Program Details:

I acknowledge that I have reviewed and understand the details of the football coaching program, including the schedule, location, and activities involved.
Health and Safety:

I certify that my child is physically fit to participate in the program and has no known medical conditions that would prevent their involvement.
I understand that football is a physical sport and involves inherent risks of injury, and I agree to hold harmless Johnathan Riley and any afilliate organization, its staff, and volunteers for any injuries that may occur during the program.
Emergency Medical Treatment:

In the event of a medical emergency, I authorize the coaching staff to seek and consent to any necessary medical treatment for my child, including emergency transportation to a medical facility if deemed necessary.
Photography and Publicity:

I grant permission for my child’s photograph or video to be taken during the program for promotional or publicity purposes.
Transportation:

I understand that transportation to and from the program is the responsibility of the participant’s family, and I will ensure that my child arrives and departs from the program safely and punctually.
Code of Conduct:

I agree that my child will adhere to the rules and code of conduct established and will demonstrate respect for coaches, teammates, opponents, and officials at all times.
Parental Involvement:

I acknowledge that my involvement and support are essential for my child’s success and development in the program, and I commit to actively participating and supporting the goals of the program.
Contact Information:

I will provide accurate and up-to-date contact information to the coaching staff for emergency purposes and communication regarding the program.
I hereby acknowledge that I have read and understood the contents of this consent form, and I voluntarily consent to my child’s participation in the football coaching program.
